clutch spring bolts

So I have been running the 14 at the dragstrip lately and am still on the original clutch since I have 3800 miles on the bike. Well last night after a 1.6 60ft and getting in to 4th it all of a sudden pegged the limiter. It would barely pull itself back to the pits because of clutch slip. I limped it home and took it apart this morning to find that the spring bolts were not tightened down. Fibers and steels are shot. Anyone else find this at a clutch change? I cant see these bolts ever backing off with any kind of spring tension on them. Somebody was sleeping at the clutch assembly line..
