I made my own using two lic. plate/inspection sticker plates. Total cost was less then $5 and it looks great.

I got the one from patricksperformance.com, got it powder coated candy red, looks great on my red bike. Makes the tag harder to read, too, +1 I had to mark the bracket for chain adjustment to mount it on the right side, though. It's designed to be mounted on the other side of the bike but I wanted it on the right.
