Stephen,

Mikes pictures should get you where you need to be.

I included a small spacer, probably 3/16ths wide. Pull out the cush drive, lay the spacer on the wheel and install the cush drive. The spacer will keep the the cush drive bearing and wheel bearing apart when tightening the axle nut.

I always installed the axle from the right side and held the spacer up with a long screwdriver and slid the axle through. You need this spacer in place.
